US Customs Forms

The following US Customs forms are available for you to view and download in a *.pdf format. They can be viewed and printed by clicking on the item you would like to view. Please find instructions for the completion of these forms below the title of the respective U.S. Customs Forms:

• Declaration for free entry of unaccompanied articles (Customs form 3299)
• Complete form as directed, for assistance please contact an International Relocation Coordinator toll free (800) 323-1963.
• Supplemental declaration for unaccompanied personal effects and household effects
• Numbers 1 through 14: All items must be completed by the owners of the goods.
• Numbers 15 and 16: Completed by National Van Lines, Inc.
• Number 17: Check "B" for Importer.
• Sign your full name as it appears on your passport and date the form.
• Customs power of attorney
• A completed and signed Power of Attorney form must accompany the 3299, to clear your shipment(s) through US Customs, this form is presented along with supplemental declaration and passport copies. Sign your full name as it appears on your passport and date the form.
